What’s Nitric Oxide (+How Does it Impact My Heart Health)?

When it comes to healthy blood vessels, nothing is more important than NO. That’s right, nitric oxide, abbreviated as NO, keeps your arteries open and even lowers inflammation.

In the air, NO is a colorless gas, naturally occurring and produced by the burning of fossil fuels. But in the human body, it does wonders.

Nitric oxide should not be confused with nitrous oxide (N2O), an anesthetic, or with nitrogen dioxide (NO2), a brown toxic gas and major air pollutant.

In mammals including humans, NO is an important cellular signaling molecule involved in many physiological and pathological processes. NO is a powerful vasodilator that opens up blood vessels. Drugs to increase nitric oxide have been used for over 100 years. Think nitroglycerin under the tongue to a patient with chest pain. Doctors didn’t even know how that drug worked!  The discovery that it was nitric oxide that resolved cardiac chest pain and lowers blood pressure led to a Nobel Prize in 1998.

 Nitric oxide in the body impacts:

1) Blood vessel function. NO leads to relaxation of smooth muscle cells in arteries, therefore, improved blood flow and lower blood pressure.

2) Smooth muscle growth

3) Platelet aggregation (sticky blood)

4) Immune cell adhesion to the endothelium ( a factor in the formation of arterial blockage)

5) Immune function as it is generated by phagocytes. NO is a free-radical that is toxic to bacteria and parasites.

How The Body Makes NO

The body makes NO in two different ways:

1) From the amino acid L-arginine, NADPH, and oxygen by an enzyme called NO synthase;

2) From nitrates contained in food

Let’s examine each pathway to the formation of NO.

Pathway number one, the L-arginine/nitric oxide synthase pathway, starts with the amino acid L-arginine. Reminder, amino acids are the building blocks for proteins. The body can make amino acids AND obtain them from protein containing foods. In the case of L-arginine, you can find plenty of this amino acid in meat, seafood, eggs, nuts, seeds, and dairy. Spinach contains the most L-arginine among the plants, but it is not much. There is another amino acid, L-citrulline, that can be converted by enzymes into L-arginine. Watermelon rind is a tremendous source of L-citrulline. Many nutritional supplements to boost nitric oxide contain a combination of L-arginine and L-citrulline. This combination allows you to use less of each individual amino acid, as they are synergistic. Oxygen is needed in this reaction. NADPH is a necessary co-factor in the formation of NO that comes from vitamin B3 (niacin). Lastly, the production of nitric oxide (NO) from the above sources requires many enzymes. These enzymes are all subject to genetic and epigenetic modification that can limit their function. Also, toxic metals and other pollutants can “gum-up” the system, leading to a decreased production of NO.

Pathway number two to NO formation is from food. Beets, spinach, arugula, and other green leafy vegetables contain nitrates (NO3). Bacteria in the mouth and gastrointestinal tract convert nitrate into nitrite (NO2). This is why is it very important to chew your food well so the oral bacteria have time to do their work. Speaking of bacteria, most Americans are using mouthwash, a product which is designed to kill oral bacteria. Bad idea. Back to nitrates to nitrites. Now that nitrites are present, they are converted into nitric oxide in various areas of the body. Myoglobin is a protein found in muscle tissue, including the heart, that converts nitrite to nitric oxide. Hemoglobin from red blood cells also performs this action. Endothelial cells that line blood vessels also contain enzymes which convert nitrite into nitric oxide, which subsequently diffuse across the endothelium into the smooth muscle cells of the artery, and ultimately lead to relaxation and improved blood flow. Additionally, salivary nitrite increases gastric mucosal blood flow and mucus production, thereby protecting the stomach.

 I Heard Nitrates Were Dangerous

For years we have been told to limit dietary nitrates and nitrites. In the 1940’s, it was noticed that well-water contaminated with high levels of nitrates from farm fertilizers led to a condition in babies called methemoglobinemia. Limits were placed on the allowable levels of nitrates in drinking water. But since that time, studies have been done that found the amount of nitrates in food is much higher than previously thought and is actually quite healthy. In fact, the human body manufactures a large percentage of nitrates/nitrites compared to dietary intake through food.

Processed meat has been vilified over the years and linked to cancer. Nitrites have been accused of being the culprit, but a 2003 review exonerated their role in cancer (WHO Food Additive series No: 50). Nitrates are added to meat and seafood products because they are very effective as a food preservative. Bacteria have difficulty growing in a high nitrite environment. Botulism is a deadly condition caused by a toxin released from bacteria in contaminated food. Sodium nitrate is often added to prevent this condition. Interestingly, nitrites added to animal food products also appear to limit the oxidation of fats and other lipids. This is a good thing.

Whether or not processed meats are healthy is a different story. Myself, I believe that processed meats from pastured animals eating their native diets are okay to eat a couple times per week. But the nitrite does not appear to be the problem and is in fact healthy. A nitrite is a nitrite is a nitrite. All get converted to nitric oxide and open up blood vessels. Many natural and organic products contain added celery juice as a preservative that is naturally high in nitrite. The celery juice was treated with bacteria to form nitrites, which is the actual preservative. Getting tired of all the nitrate/nitrite conversation yet?

Finally, some processed meat, beer, and tobacco lead to the formation of nitrosamines. These can cause cancer. The good news is, vitamin C has been shown to limit the production of nitrosamines (Am J Clin Nutr 1991. 53:1). Veggies along with your pasture raised bacon should do the trick.

 Boost NO Naturally

Pathway number one can be boosted with the intake of L-arginine and L-citrulline containing food. The NADPH necessary in pathway one relies on ingested niacin for its formation. The enzymes that eventually produce NO from the above ingredients depend on magnesium and other nutrients. It takes a village of vitamins and minerals to stay healthy. There is plenty of evidence that L-arginine supplementation works. A meta-analysis from 2011 demonstrated the efficacy of L-arginine supplementation and blood pressure control Am Heart J. 2011 Dec;162(6):959-65.

Pathway number two relies on nitrate intake from food. This is where beetroot shines. This red root veggie is loaded with nitrates. Pretty much all green leafy vegetables contain nitrates. Unfortunately, cooking reduces the level of nitrates in food.

Plenty of studies have been performed using beetroot juice. Beetroot juice lowers blood pressure and has many other health benefits including anti-oxidant and anti-inflammatory. A study from 2008 found that beetroot lowered blood pressure by 10 points, decreased the stickiness of platelets, and improved the function on the endothelial cells that line blood vessels.  Test Your Levels

In my office, I love to test the nitric oxide level of my patients. It is a simple test strip that checks the saliva NO in just a couple of seconds. Results are instantaneous and easily reproducible. The patient changes their diet, lifestyle, and supplements and the test is repeated in a month.

Heart Health Benefits of Probiotics


Probiotics may be the most important supplement of all. Every one of my cardiology patients MUST take them. Their success (and mine) depends on it. Probiotics literally means “pro-life” and antibiotic means “anti-life”. Doctors use the term probiotic to encompass the trillions of bacteria (flora) and beneficial yeast that colonize our intestines. The gastrointestinal (GI) tract has a delicate balance of beneficial and harmful microbes doing the dance every day. When this coexistence gets out of whack in favor of the bad guys, disease will flourish.

Heart Health Benefits of Probiotics

The beneficial bacteria in our GI tract are responsible for:

  • Digestion
  • Vitamin synthesis
  • Detoxification of chemicals
  • Immune support
  • GI permeability (keeping unwanted particles out of the body while letting beneficial ones in)

Poor nutrition from grain (especially gluten containing grains), dairy, and sugar wreak havoc on the gut and can allow the bad bacteria and fungi to flourish. Antibiotics, steroids, fluoride, and chlorine inflict major GI damage.

How to maintain a healthy gut

If you do not have proper digestion with adequate amounts of stomach acid and digestive enzymes, disease will begin. Our ancestors ate bacteria in the dirt every day. They didn’t pull a carrot and scrub it clean. You can imagine Paleo people had very dirty hands, which would actually boost the immune system.

The health benefits of probiotics have been recognized for thousand of years. Our ancestors may not have known about the tiny bacteria themselves, but they knew the healing properties of fermented foods such as sauerkraut, kimchi, kefir, and yogurt. One bite of these items has billions of good bacteria and I recommend eating kraut or kimchi weekly. Kombucha tea would also be an excellent source of probiotics and is great for an afternoon pick-up. Dairy is best left for the baby cows.

WHY DOES A CARDIOLOGIST CARE ABOUT THIS ISSUE?

Well, cardiovascular disease is linked to inflammation and a major source of inflammation is the gut. Heal the gut and heal the heart. The good news is, probiotics are proven to heal the leaky gut.

Hundreds of studies confirm the benefits of probiotics on cholesterol levels. A study in the European Journal of Clinical Nutrition found those patients randomized to probiotics vs. placebo increased there HDL from 50 to 62.

Probiotics reduce blood pressure according to numerous studies and my clinical experience. Probiotics improve blood sugar. The lower the blood sugar, the lower the risk of heart disease. Probiotics lower inflammation and improve heart function!

In addition, probiotics:

  • Aid in food digestion and may reduce acid-reflux symptoms
  • Enhance the synthesis of B vitamins and improve calcium absorption
  • Improve symptoms of irritable bowel syndrome and ulcerative colitis

PROBIOTICS are best consumed on an empty stomach so I recommend dosing when you first wake up and 30 minutes before dinner or even at bedtime. It is good to take a variety of probiotics as there are hundreds of different strains that make up a healthy gastrointestinal tract. Overdosing is very unlikely, if not impossible. Side effects are mostly limited to loose stools.

When our ancestors wanted a carrot, they would pull one out of the ground and eat. In present day, our store bought vegetables are washed before we buy them and then scrubbed at home to get off any remnants of dirt. I am not opposed to the cleaning of our food due to toxic soil, air, and water, but this is all the more reason to consume probiotics on a daily basis. In addition, our hands used to be dirty from the soil and our hunter-gatherer lives. Not any longer, you can’t walk five feet without running into hand sanitizer which effectively removes all good probiotics along with giving you an extra dose of chemicals. Could the diseases afflicting our children and the recurrent infections we all seem to get be due to our sterilized society?

Top 10 Ways to Detox Vaccines

1. Water

Without water, life on earth does not exist. This is the reason water is first on our list.

By drinking quality water, you are going to flush out the pollutants through your kidneys and colon. Via urine, your body discharges all kinds of contaminants including metals, plastic, phthalates, and hundreds of others.

Your liver makes thousands of life-sustaining molecules. It also performs double-duty as a major detoxifier and cleanser of the body. From the liver, bile is secreted, fills the gall bladder, and dumps the good and the bad into the intestines.

10 Foods to Boost Nitric Oxide


One of the most important molecules for blood vessel health is nitric oxide (NO). It is a major vasodilator, as it keeps blood vessels open. Doctors have prescribed nitroglycerin to patients for many years for patients with chronic chest pain (angina pectoris). Presidents Dwight Eisenhower and Lyndon Johnson lived on the stuff. Nitroglycerin gets converted to nitric oxide.

Our Body Makes NO- if we give it the tools it needs.

There are two cellular pathways in the body that generate nitric oxide. Dietary sources of nitrates and nitrites enter the body through the gastrointestinal tract and are subsequently converted into nitric oxide. In a second pathway, an enzyme known as nitric oxide synthase (NOS) synthesizes nitric oxide from the amino acid L-arginine and oxygen.   1. Dark Chocolate

Guess what, chocolate is healthy. Well, that is only partially true, but at least we got your attention. The raw cacao bean increases nitric oxide and is loaded with antioxidants. Cacao can lower blood pressure and markers of inflammation. Unfortunately, chocolate is loaded with sugar. Save it for special occasions and skip the milk chocolate. Only go for the dairy-free dark variety. I like to add raw cacao to my breakfast “cereal” of nuts and seeds along with coconut flakes and homemade nut milk.

  1. Citrus

Oranges, lemons, and grapefruit contain high amounts of vitamin C, which has been shown to protect your precious nitric oxide molecules from free radicals. Vitamin C from any source raises levels of nitric oxide synthase, the enzyme that converts L-arginine into nitric oxide. Vitamin C is also a co-factor in reducing dietary nitrite to nitric oxide.

  1. Pomegranate

This delicious fruit boosts nitric oxide and is a tremendous anti-inflammatory. It also reduces oxidative stress, a leading factor in the production of coronary artery disease. Polyphenols in the pomegranate assist in converting dietary nitrite to nitric oxide. These same polyphenols block nitric oxide from converting back to nitrite.

Pomegranate inhibits the formation of monocyte chemoattractant protein, a molecule that recruits inflammatory cells to the blood vessel lining. This is a major factor in coronary artery disease. 4. Walnuts

Most people know that walnuts are high in heart healthy vitamin E. But because of their high amount of L-arginine, walnuts keep the blood vessels running freely. Interestingly, walnuts look a lot like the human brain, so eat them for brain health as well. Most other nuts are a good source of arginine. Soak your nuts for six hours prior to using.

5. Arugula

Also known, as rocket lettuce, arugula is the highest source of nitrates known. This bitter green is perfect in salads or sautéed with other veggies. We mix with grilled onions and use on top of our grass-fed burgers. I usually eat it straight out of the bag in handfuls.

6. Spinach

Want to be strong like Popeye? Eat your spinach (but fresh, not out of the can). This leafy green is packed with nutrients, and of course, nitrates. Add to salads, soups, sautéed or just straight out of the garden.

  1. Watermelon

Watermelon is loaded with the amino acid, L-citrulline, which gets converted into L-arginine and ultimately nitric oxide. So many people reach for L-arginine supplements, but the body does not absorb it well. L-citrulline is easy absorbed.

8. Beets
